There is no such thing. Either you have a disability or you do not. Either your dog is a trained Service Dog, or they are not. 

According to the ADA regulations, there is no certification, identification, or documentation required for your Service Dog. Places of business are prohibited from asking for it. Neither are you required to carry any form of documentation regarding your disability.

In case you get separated, it is a good idea to have a personalized dog tag. My dogs have a tag that identify them as a "Service Dog", my cell phone, and my home number. You can make these yourself, for just a few dollars at almost any place that carries pet supplies.

I use a vest or badge on my service dog, and highly recommend their use. It is helpful not only to businesses, but lets the public know that your dog is on the job, so not to interrupt or distract them from their duties.

Micro-chip and register your dog! In case your dog slips their collar, or someone removes it. A micro-chip is forever. Animal Shelters and Veterinarians have the ability to check for micro-chips.

AIRLINES

I have traveled with my service dog on airlines. They do not require tags hanging from your dog. Check with the airline you will be traveling on, for their specific requirements and let them know ahead of time if you are bringing a service or emotional support dog.
